Has anyone used a PS plug-in or add-on for masking that they like better than PS? With the ability to mask fine detail like hair etc.

Yes, I have used Mask Pro. Most anything you get is better than using Image Extract or the Quick Mask Tool. They 3rd party tools have more options for the masking and the fixing up of the mask once you get your starting part. There is no holy grail when it comes to masking IMO.

LivePicture or Mask Pro. (Depending on your platform.)

knockout v2 does a pretty decent job, even with hair. It's available as a plug for PS
