If you happen to have access to Photoshop CS, in the Filter Gallery there is a new "Grain Filter" that is heavily adjustable. If you do not have access to CS drop me a note and I will send you something from that filter if you can give me an idea of what you want.

P.S. You can get a fairly good approximation from the "noise" filters in the earlier version of PS as I did below:
