The main problem I see here is the hand is amputated and the crop is too tight. Loosening the crop a little would have allowed the whole hand to show and put the eyes in a better position in the image. There is nothing wrong with cropping the head if you are doing it to follow the rule of thirds and put the eyes in the proper position. In this image it looks like her lips were the main object of the photo, but usually in a portrait you want to draw attention to the eyes. The lighting and pose look pretty good.

Agree with the cropping but JUST IMO I like to see the face and eyes. If it were my shot I would have lit up the face just a little more. Not a huge fan of shadow on a portrait. Just my 2 cents

If I would to make another suggestion, it looks like the highlights on her are are really the brightest part of image and my eyes are drawn right to them, which probably isn't the intended focal point.

the even 45 degree x 45 degree lighting on each side doesn't work with her hair style. Both lights are casting a shadow strait into the eye and making them look dark and dead. In this case I would have gone with one light high behind me and one light low right in front of me about a stop lower in power for fill.
Something like this is what I'm talking about...
