I designed my own model information form about 10 years ago using CorelDraw. At the time, I'd never heard of any other photographers doing anything like that.

My form is much like an 'Employment Application' in format and tone although it does ask several questions pertinent to modelling and includes spaces for my own interview notes and info such as ID (driver's licence number). I keep a file folder for each model I use and the form is filed first in the folder. Other stuff such as their headshot or comp card, model releases, a copy of any copyright license I give them and other shooting session info is filed in the same folder.

I don't have my form in any shareable format. In any case, i recommend you design your own that asks the questions you need answered. I've changed mine from time to time, taking stuff off that I really didn't need and adding other questions I felt were important.

I've been using this form for years and found it collects everything I need to book a model.

At every shoot I get the models to sign a Model Release which collects name, DOB, address, etc. If I plan to submit the model to any magazines, I also get a copy of their drivers license and ask them to provide me with information like education, hobbies, awards, occupation, or anything I think the magazine might like to know about her.

I found something at:http://forms5.createonlineforms.com/5649/form_1_1.html
that looks pretty good. When I copied it into MS Word I had to change the formating
from Normal + Arial Black Bold Centered to Normal and then I could read it..
