How do you achieve this look?

Probably some or a lot of the above - and - I come to think of a Photographer/Photoshop Artist I know of in Stockholm who shoots a lot with Bluescreen.
... or - in his case, Greenscreen.

Is it done with lighting or mostly in Photoshop? & if it is done in PS, what are the likely steps to achieve a similar look?

At first glance it looks to me like a little of each. Clearly there is some kind of portable flash unit used, actually looks like two one from the front and one from the side with a higher output giving the right side of her that extra highlight. Using some tanning oil or baby oil on the skin will give the model's skin that nice shiny effect. The skin on her face has clearly been retouched and it looks to me like her eyes and lips have been sharpened.

He did? Really sad. I met him once. Real nice guy.

I don't know if you know him well, or if I'm prying or anything, but was he sick or anything? He seemed perfectly healthy when I met him, which was just a few years ago.

Sam, it was an apparant suicide from what I have heard...There were a few threads on other boards about it...
