How do I achieve the "glow"

I'm not sure I follow your question. This looks like it's diffused, like maybe with a technique similar to Rolando's "black scarf" trick (look in the photo tips section). The color is very, very warm, but that suits the shot. Beyond that, I'm not sure what you mean by "glow".

The warm tone to the skin. Is that done in PS or by use of a filter/gel etc.?

It is done several ways. Gels and Lights along with Veg OIl, Pam or baby oil for the body shine.

The face is lit with a separate light with possibly a gel. I would guess a CTO gel. On playboys shoots they may use as many as 20 lights or more with some having gels. White balance is also a factor in the setups. Using a warm WB will have this effect also.

This has already been pretty well answered. You might look through some of Rolando's posts, as he uses gels a lot. Nice thing about gels is you can apply the color to just a portion of the image, whereas a lens filter affects everything.

Also, as someone mentioned, she's been oiled up pretty well. Baby oil gel works pretty good. Smells nice too.
