I'd second or third the vote on an optical multi-button and a Wacom Tablet. Look at the Graphire 3 range, half the price of the Intuos and the only thing it lacks if the angle sentitivity.
I have a Graphire 3 6x7 and a Microsoft Optical mouse, I use the mouse with my right hand (buttons and wheel scrolls) and the pen in my left (being ambidextrous does have an advantage with this), dual monitors (19" for the photo and 17" for the tools) and it make life a lot easier doing manipulating the photos.
