Position of the light

Hi,
I'm an amateur photographer and still much to learn....therefore I;m not satisfied with my photos and start to think about position of the light, specially the main light.
I do not know what should be the teoretical best position of the main light that I should look for.
Here I show some of testing photos I did. These photos are just as were taken, without any sinlge change-directly from camera just resized. Camera Canon350D.
For the first photos I was using as main light Beauty dish 40cm on 600w, backlight on 60x40cm softbox on 300w and 430ex flash for camera left backlight. For the last photos I was using octagon 120mm as main light...Small room 6m with 3,5m with lot of furniture.
Question: Where are my mistakes, how should I know that I position my main light propretly.
Question 2. when I set up the light what is my begining: first I set up the main light or first I do the backlight....what are the steps to do it right.
For example, the last photo I show have the problem with highspot on her head... and my camera doesnot making any critical point on histogram.... How should I adjust the light to avoid it.....
