Okay, I figured it out.
The "body sculpting" layer is the amalgamation of the layers below. Thus, the author forgot to tell the reader to turn off the prior layers, with the one exception of the white background layer.
Now the reader is supposed to paint black on the on the layer mask. That hides the portion that is "cut away" and allows the white background to show through.
